Application Agreement Authorization, Release and Waiver
I have read, understand and agree to the TJJ standards of conduct (www.ncsy.org/standards) and I agree to scrupulously uphold these standards throughout my participation on the program.
I understand that this program is conducted in consonance with TJJ's religious and educational standards. As such, I agree to adhere to the program and behave in a manner reflecting positively upon TJJ and my family. I agree to dress modestly throughout the summer in compliance with the TJJ dress code requests for TJJ for Moms and/or TJJ for Dads. I understand that all TJJ program participants must travel and remain with their assigned groups at all times.
I am aware that I will be held responsible for any damage to public or private property that TJJ states I caused as a Summer Program participant, and I agree to fully reimburse all parties involved. I acknowledge that I may never use or be in the possession of narcotics during the Summer Program, and TJJ may request at any time that I voluntarily undertake a non-physician administered test for drugs and narcotics. Refusal to take such a test voluntarily or a positive result obtained from such a test will serve as grounds for immediate dismissal.
I further affirm my understanding that my acceptance to TJJ for Moms and TJJ for Dads summer programs is contingent upon a successful interview. Acceptances are at the sole discretion of the Director of TJJ for Moms and TJJ for Dads.
ORTHODOX UNION MEDIA RELEASE
I hereby authorize the Union of Orthodox Jewish Congregations of America, TJJ for Moms and TJJ for Dads and its affiliates, employees and agents, (together, “TJJ ”) to take photographs and/or video of me when I participate at TJJ events. I grant TJJ full rights to use and share the images resulting from the photographs and/or videos and any reproductions thereof (the “Recording”), in whole or in part, along with my name, biographical information and quotations for any purpose including fundraising, publicity, advertising, or on social media, all without further obligation or compensation. I hereby waive any right of review or approval regarding the Recording and TJJ shall have the right to edit or delete the Recording in its sole discretion. I understand that all rights in and to the Recording shall be TJJ’s sole and absolute property. I understand that if I access TJJ’s services through a third party platform, product, service, or application (a “Third Party Service”), content and related information generated while accessing TJJ’s services in this manner may be monitored, captured, and or recorded by the third parties that provide such Third Party Services. I consent to the use and sharing of personal information by any such third party as described in the privacy policy available through the Third Party Service. I release and forever discharge TJJ from all causes of action, suits, or liability of any kind arising from or in connection with the use or publication of any Recording of me made in connection with my participation in TJJ events. I waive any right of privacy associated with the Recording and I waive any right to compensation related to the use of the Recording.
I am entering this agreement on behalf of myself, and understand this agreement will be binding on me. I have read this release and I fully understand its contents.
I have read this release and waiver and I fully understand its contents. In consideration of the acceptance of myself to the TJJ Summer Program for which I have applied, I do hereby agree to the six items listed below:
1. Compliance: I shall comply with all rules, regulations and standards of conduct fixed by TJJ, its agents and employees. I acknowledge and understand that the Director of TJJ for Moms and TJJ for Dads participation if s/he deems it necessary, in its sole discretion, for the best interest of the program. In the event I decide to terminate my participation prior to the program's conclusion, I will assume all responsibility to coordinate my travel arrangements to return home. In either of these cases, I understand that any and all expenses associated with the termination will be my sole responsibility and I understand and agree that no refunds will be issued.
2. Cancellation and Withdrawal: Please see our Pricing & Terms page (http://summer.ncsy.org/pricing-and-terms/) for our full policies of Withdrawals, Refunds and Cancellations.
All flight deviation or modification fees are the responsibility of parents of program participants. TJJ is not responsible for the cancellation of any flights or any fees connected with such cancellation or lost or damaged baggage. In all cases of lost or damaged luggage, program participants must deal directly with the applicable airline to seek any recoveries or compensation.
3. TJJ Summer Program fees include food and accommodations for the duration of the trip and international group travel.
All program dates and fees are subject to change due to flight and fuel price increases and other variables. Additionally, the duration, itinerary or other details of the program may be canceled, curtailed, altered, or modified (any of the foregoing, a Program Modification), at the sole discretion of TJJ, for any reason, without prior notice, obligation, or reimbursement. For the avoidance of doubt, for TJJSummer Programs traveling outside the United States, the specific reasons for any such Program Modification may include, but are not limited to, changes in prevailing safety and travel risks, as determined in the sole discretion of TJJ.
By checking the box, I release, TJJ and the OU, their respective officers, directors, trustees, affiliates, agents, employees, independent contractors, consultants, advisors, vendors, volunteers, licensees and assignees (collectively “Releasees” referred to herein and each a “Releasee”) from and against every claim, loss, damage and liability or responsibility, including, without limitation, reasonable attorneys’ fees and costs (“Liabilities”) arising from any judgments or decisions made in the obtaining and rendering of medical assistance and treatment for me.

5. Travel Awareness:
If I am traveling overseas with TJJ, I affirm my understanding of the risks associated with international travel. By checking the box located below, I affirm for myself and on behalf of my family, next of kin, heirs and representatives, that we shall make no claim against TJJ, the OU, their officers, trustees, agents, employees, independent contractors, program suppliers, hotel, common carrier, travel agency, licensees and assignees, for any damage, injury, death, loss or expense resulting from overseas travel in connection with my attendance at, and participation on, a TJJ Summer Program.
I further understand that TJJ and the OU shall, in no way, be deemed responsible for the operation or management of any means of transportation, public or private, or facilities used or enjoyed by the program. Additionally, any claim which may accrue to me against the airline carrier providing transportation between the country of origin and Israel and/or Europe and return will be limited by the Convention of Warsaw of October 12, 1929, as amended, and any other applicable domestic international laws or treaties, the terms of the Charter Agreement with the applicable airline carrier(s), and published tariff regulations.
6. Israel Travel Considerations (applicable only to participants in TJJ Summer Programs traveling in Israel): By registering myself and/or participating in all or part of any trips or activities organized by the OU, including any travel to, from and within Israel, I agree to the following:

These numbers are available from 8:00 a.m. to 8:00 p.m. Eastern Time, Monday through Friday (except U.S. federal holidays).
I have read and understand the aforementioned Travel Warning, including specific subsections relating to risks particular to Jerusalem, Northern Israel and the Golan Heights, the West Bank, and Entry/Exit Difficulties, and I nevertheless permit myself to attend the TJJ Summer Programs that travel to Israel. I accept all risks associated with the heightened dangers of travel to, from, and within Israel in connection with the TJJ Summer Programs that travel to Israel.
I understand and agree that none of TJJ, the OU, their officers, trustees, agents, employees, independent contractors, program suppliers, hotel, common carrier, travel agency, licensees and assignees, shall be responsible or liable, and I, for myself and on behalf of my family, next of kin, heirs and representatives, shall make no claim against them or their officers, trustees, agents, employees, independent contractors, program suppliers, hotel, common carrier, travel agency, licensees and assignees, for any damages, injury, death, loss or expense resulting from travel or in connection with my participation on the TJJ for Moms and/or TJJ for Dads Summer Program.
I HAVE READ THIS AGREEMENT AND AM FULLY AWARE OF ITS CONTENTS, INCLUDING THE RELEASE THAT I HAVE AGREED TO HEREIN.
